Do You Choose Tibial Fit, Tibial Rotation, or Both?

old-man-running.jpg

Who says you can not have your cake and eat it too? Why should a surgeon and their patients have to settle for unacceptable results with regards to selecting rotation and fit of their tibial implant?

In Dr. Gregory Martin’s attached paper titled, “In-Vivo Tibial Fit Analysis of Customized, Patient-Specific TKA Versus Off-the-Shelf TKA” which was presented at the 2014 Transatlantic ICJR Meeting, he found that it is possible to get great fit and rotation of the tibial implant.

In every knee replacement, a surgeon must choose between maximizing the fit of the tibial implant and sacrificing rotation or accepting poor tibial coverage and achieving proper rotation. Dr. Martin has proven that Conformis has made these compromises obsolete.

A custom-made knee replacement, which Conformis is the only company in the world to offer, respects the shape of the patient’s native tibia and delivers an implant that maximizes coverage and provides accurate rotation.

Dr. Martin informs us that the advantages of a custom made tibial implant include a lower incidence of knee pain from a tibial component that overhangs, implant loosening due to poor bone support, and ultimately poor patient satisfaction.

It is known that 20% of patients are not satisfied with the outcomes of their knee replacements. Not many industries can survive for long with such high levels of unsatisfactory results. Around 700,000 knee replacement procedures were performed in 2017. This means there are approximately 140,000 patients who are unsatisfied with their results. With such poor subjective outcomes, why not consider a technology that addresses many of the root causes of patient dissatisfaction? The answer is not to set low expectations for the patient but rather to meet or exceed a patient’s expectations. Check out this fit analysis article by Dr. Martin comparing customized, patient-specific TKA versus off-the-shelf TKX.

Patient satisfaction in total knee replacement can be improved through the use of customized knee technology. To learn more about Conformis Knee Replacement in NYC and other orthopedic solutions, contact Empire Surgical today.

Father of Snowboarding & Conformis Recipient Jake Burton

Uploaded by Conformis, Inc. on 2018-04-18.

The father of snowboarding Jake Burton is a “Proud owner of the Conformis knee.” In this video, he mentions that other people may have had knee replacements and have pain but he wisely states that they got the wrong knee or the wrong doctor. He touts his Conformis Custom Made Knee Replacement and his excellent surgeon Dr. Bryan Huber.

Dr. Huber notes how Conformis allows him to have conversations with his patients that include the uniqueness of their knee, their offsets, and where their deformities are. These are not the conversations that a surgeon would have with a patient who is receiving off-the-shelf knee replacements because off-the-shelf knee replacements do not respect the individuality of each patient's unique anatomy. It does not need to be about setting a patient’s expectations when there is an option to meet a patient’s expectations.

Post surgery, Mr. Burton went home within 24-48 hours, was walking independently in a couple of days, went to the gym in one week, and was snowboarding at three weeks.

Mr. Burton wanted a knee that would work and that is exactly what he got. Keep in mind that a knee that works for Mr. Burton is a lot different than how most orthopedic surgeon would define a knee replacement that works.

The father of snowboarding is back doing what he should be doing thanks to Dr. Huber, Mr. Burton’s determination, Conformis, and the rest of his team. His most grandiose expectations were met!

How to Make a Custom Knee Replacement

The video below shows a summary of the level of precision that goes into the design of every custom made Conformis Total Knee Replacement. It all starts with a cat scan of the hip, knee, and ankle which is used to ensure the patient is going to receive a knee that is aligned neutral to mechanical axis aka straight. Then the Conformis CAD software maps out the unique shape of the patient's knee, corrected for deformity, being sure to respect the natural condylar offsets of the individual's knee.

Once the femur has been mapped out, the CAD software and engineers get to work on the tibia. Just like with the femur, each tibia has a custom-made shape that matches the unique profile of the patient's anatomy.

The final result is a knee that is a perfect fit and perfect shape. Patients who receive a Conformis Custom-Made Knee Replacement are far more likely to be satisfied with the outcome of their procedure.

USA Today Video Featuring the Conformis iTotal

The video below features Dr. Bryan Huber of Morrisville and his patient Dalton Harben. Mr. Harben was involved in a gruesome skiing accident that occurred when both of his skis fell off. He went sliding down the mountain hitting rocks and trees which resulted in him requiring seven surgeries. At one point he was even told that due to the cumulative insults of his injuries his best option was an amputation.

Fortunately for Mr. Harben, he was able to get in touch with Dr. Huber who was able to work with the Conformis engineering team to design a custom knee replacement which recreated his unique anatomic geometry and gave him back a perfectly straight knee which matched his unique condylar geometry perfectly. This not only saved him from amputation but also restored his quality of life.
